I’m so excited to be working with you on your design project! To make the best use of our time together during your design consultation, I have prepared the following questionnaire so that I can get a good idea of your lifestyle and any project goals you may have for our work together.

Please answer as many questions as possible and submit this questionnaire at least 48 hrs prior to your consultation.

We’ll review your answers and delve deeper at your consultation meeting, as appropriate.
